Japan’s statutory health insurance system (SHIS) is government regulated universal
healthcare that provides healthcare coverage to 98.3 percent of Japan’s population, and is funded
by taxes and individual contributions. It is mandatory for all Japanese citizens to enroll in either
an employment-based plan or a residence-based plan. The national government regulates nearly
all aspects of the insurance system and enforces detailed regulations for insurers and providers.

In the United States there is no “universal” structure for healthcare, however, when the
Affordable Care Act (ACA) passed in 2010 it increased the government’s role in financing and
regulating healthcare. The United States healthcare system is a mix of employer-sponsored
healthcare coverage, private pay insurance, and public programs such as Medicare, Medicaid, the
Children’s Health Insurance Program (CHIP), and the Veteran’s healthcare program
TRICARE.Medicare is funded by payroll tax shared by employees and employees, and covers
individuals age 65 and older, as well as people younger than 65 who have long-term disabilities.
Medicaid is funded by federal and state taxes and provides healthcare to adults and children in
low-income families, the blind, and people with disabilities. CHIP is also a public program that
provides healthcare coverage for children in low-income families that do not qualify for
