started two weeks ago. The pain is accompanied by shortness of breath, yet both are relieved at rest. She states these
episodes occur two to three times a week. She has a history of hypertension and hypercholesterolemia. She is a
former smoker and has a family history of heart disease. Her lifestyle includes regular exercise, daily wine, and better
diet choices.

CC: chief complaint is chest pain accompanied with shortness of breath that radiates to left arm and resolves with rest
beginning two weeks ago while skiing in cold weather 5 degree F
HPI: F.B. is a 66 year old female presenting with chest pain that began two weeks ago while skiing in 5 degree
weather. work life unchanged however lifestyle changes include exercises up to 3 times per week and better food
choices in restaurants.

Review of Systems:
General: Generally, feels in good health. Denies: fatigue, difficulty sleeping, unintentional weight loss, unintentional
weight gain, fevers, and night sweats.


HEENT/Neck: Denies: headaches, epistaxis, dizziness, blurry vision, problems hearing, ear pain, sinus problems,
chronic sore throats, or dysphagia. Denies drainage from ears or eyes or nasal polyps.


Cardiovascular: denies palpitations. denies history of arrythmias, edema or claudication. complains of midsternal chest
pain upon exertion radiating to left arm beginning two weeks ago relieved with rest; occurring two to three times a
week.
