colonies growing on Middlebrook 7H11 agar. These colonies are
commonly recovered from expectorated or induced sputum
specimens obtained from patients with signs and symptoms of
pneumonia. The appearance of these colonies provide for a
presumptive identification, supported by the positive yellow-
pigmented niacin reaction seen in the reagent tube in the lower
photograph. From these observations, select from the multiple
choices the presumptive identification of this isolate.
Mycobacterium tuberculosis
Mycobacterium tuberculosis
requires 10 days or more of incubation for growth. It is
characteristically described as a rough, non-pigmented or buff
colony. It also produces a rapid positive nitrate test reaction.
Reactivation adult tuberculosis is a slowly progressive inflammatory
process in the lungs, with intense granulomatous inflammation and
cavity formation.
Mycobacterium fortuitum
colonies are non- pigmented with a smooth rather than rough
surface. M. fortuitum can be presumptively identified by it's more
rapid growth within 3 - 5 days, and observation of a negative niacin
accumulation reaction. Recovery in culture is most commonly
considered a non-pathogenic commensal.
Mycobacterium kansasii
colony growth is also prolonged and the growing colonies may also
appear roughened. Being one of the photochromogenic
mycobacteria, the colonies would take on a yellow pigmentation

, after exposure to light. The niacin accumulation reaction is also
negative for M. kansasii. Recovery from upper respiratory
specimens may also be experienced with M. kansasii but can be
presumptively differentiated from M. tuberculosis by the colony
appearance and differing biochemical activity.
Mycobacterium gordonae
colonies are also slow growing on mycobacteria culture media, and
have smooth surfaces that are distinctly yellow-orange pigmented
even prior to exposure to light (scotochromogen). The niacin
reaction is negative. M. gordonae is uncommonly recovered in
routine laboratory cultures, but may be recovered from aqueous
environments, colloquially referred to as the "tap water bacillus".
