NUR 106 Module E Practice Questions
and Correct Answers
Newborn Care ✅From chord first being cut the next two to four hours are critical and
called the transition period

Maternal History ✅Medical history of mom, medications/substances taken during
pregnancy. Need to know early because it can prevent further gestational problems.
Prenatal care, previous OB history-issues?
Maternal labs- blood type, infections, glucose

Labor experience ✅When did fluids break? Color? Tells you about infection.
Medications given during labor- for pain, antibiotics, for glucose levels?
Fetal heart rate pattern- how did baby respond to labor?
Nurse needs to know previous labs- Blood type, RH factor, infection status-HepB, HIV,
strep
Any previous pregnancy problems?
How long was pregnancy?

Respiratory development ✅Most important development after birth.
Longest to develop/manifest besides CNS
During birth the lungs are filled with fluid, birth canal squeezes the fluid out and causes
a recoil, which in turn stimulates baby to take a breath.
Surfactant is a necessity for lung formation- it lines alveoli so they don't collapse; the
baby has very little once they are born, after the first few breaths, the little bit of
surfactant is used.
Surfactant increases surface area for gas exchange and make the next breath easier;
it's produced at 22 weeks during gestation but it's produced very slow.
Surfactant doesn't mature until 37 to 38 weeks.

Increased surfactant production ✅Stress of labor
HTN in mom
Corticosteroids given to help mature lungs, and is usually used to speed the surfactant
production when a baby is being conceived early.

Decreased Surfactant Production ✅Infections in mom
After birth cold stress
Increased glucose

Mild hypoxia and acidosis ✅To fix, body compensates by blowing off CO2 during fetal
period

Chest compression ✅Gets rid of fluid and lets in breath

, Temperature immediately after birth ✅100 degrees

When baby cries ✅You know he's breathing, if the baby is just breathing and not
crying that's okay as well

Drying infant ✅1. Stimulates respirations
2. Gets rid of fluid, so don't lose current body temperature

AIRWAY (critical) ✅Position with head slightly extended, if lying down the nose will be
directly in line with ceiling. Sometimes called the sniffing position.
It's the FIRST most CRITICAL step.

After airway, suctioning ✅Only suction when necessary, start least invasive with bulb
syringe.
Bulb syringe is squeezed first and then placed at the sides of the baby's mouth to
suction.
You are trying to clean oropharynx and nasopharynx.
Suction mouth first and then nose. >makes baby angry, so if they cry they will exhale
through the mouth, and you'll have already suctioned there. (So you don't elicit the gag
reflex= and the baby doesn't stop breathing)

Assessment immediately after birth ✅Observe
Auscultate (if possible)
Palpitation
Is the baby breathing with adequate effort?

Assessment cont'd: Best way to tell if the baby is well oxygenated? ✅Don't look at the
color of the baby, their heart rate will be the most accurate way of determining
oxygenation. The count should be at or above 100 beats per minute.
Do not count for a full minute, do not count for 30 or 15 seconds either, count for 6
seconds and multiply by 10 (to get your full minute calculation).
Reminder, all of these things are happening seconds after the baby is born, these
assessments have to be done QUICK.

How do you know when the PAO2 is increasing in the newborn? ✅5 to 10 minutes of
adequate breathing

Breathing ✅You only need to know if they're breathing or not, you can palpate in
between chest and abdomen to check if they are. Babies are more abdominal breathers
after birth.

How do you know if the respiratory rate is normal in the newborn? ✅Up until 2-3
months the respiratory rate will be irregular
