Hand washing - ANS 1. address client by name and introduce self to client
2. turn on water at sink
3. wet hands and wrist thoroughly
4. apply soap to hands
o dry , hands, and fingers producing friction, for at least 20 seconds, keeping hands lower than
the elbows and fingertips down
6. clean fingernails by rubbing fingertips against palms of the opposite hands
7. rinse all surfaces of wrists, hands, and fingers keeping hands lower than the elbows and the
fingertips down
8. use clean/dry paper towels to dry all surfaces of hand, fingers, wrist then dispose of paper in
waste container
9. use clean/dry paper towel to turn off faucet then dispose of it in garbage
10. do not touch inside of sink anytime
Applying one knee-high elastic stocking - ANS 1. explain procedure, speak clearly, slowly,
directly, maintaining face-to-face contact whenever possible
2. provide privacy
3. client in supine position while stocking is applied
4. turn stocking inside-out, at least to the heel
5. places foot of stocking over toes, foot, and heel
6. pulls top of stocking over foot, heel, and leg
7. move foot and leg gently and naturally, avoiding force and over-extension of limb and joints
8. finishes procedure with no twists or wrinkles and heel of stocking, if present, is over heal and
opening in toe area (if present) is either over or under toe area
9. signal light in reach; bed in low position
10. once done, wash hands
Assists to ambulate using transfer belt - ANS 1. explain procedure, speak clearly, slowly,
directly, maintaining face-to-face contact whenever possible
2. before assisting to stand, client is wearing shoes
3. before assisting to stand, bed is at safe level
4. before assisting to stand, check and/or locks bed wheels
5. before assisting to stand, client is assisted to sitting position with feet flat on the floor
6. before assisting to stand, apply transfer belt securely over clothing
7. before assisting to stand, provide instructions to enable client to assist in standing including
prearranged signal to alert client to begin standing (1, 2, 3)
8. stands facing client, count to 3 to alert client to begin standing
9. on 3, gradually assist client to stand by grasping transfer belt on both sides with an upward
grasp and maintaining stability of clients legs
10. walks slightly behind and to one side for a distance of 10 ft, while holding onto the belt
, 11. after walking, assist client to bed and remove transfer belt
12. signal device in reach
13. wash hands
Assists with use of bedpan - ANS 1. explain procedure, speak clearly, slowly, directly,
maintaining face-to-face contact whenever possible
2. provide privacy
3. before placing bedpan, lower head of bed
4. put on clean gloves
5. place bedpan correctly under client's buttocks
6. remove and dispose of gloves into waste container
7. wash hands
8. raise head of bed
9. toilet paper in reach
10. hand wipe in reach and client is instructed to clean hands with wipe when finished
11. signal device in reach and client asked to signal when done
12. put on clean gloves
13. lower head of bed.
14. avoid overexposure and remove bedpan
15. empties and rinses bedpan and pours rinse into toilet
16. place bed

17. remove and dispose of gloves
18. wash hands
19. signal device in reach
20. bed in low position
Cleans upper or lower dentures - ANS 1. puts on clean gloves before handling dentures
2. line bottom of sink with cloth
3. rinse dentures in moderate temperature running water before brushing them
4. apply toothpaste to toothbrush
5. brush surfaces of dentures
6. rinses surfaces of dentures under moderate temperature running water
7. rinse denture cup and lid
8. place denture in cup with moderate temperature water/ solution and place lid on cup
9. rinse toothbrush and place in designated toothbrush basin/container
10. maintain clean technique with placement of toothbrush and denture
11. sink liner is removed and disposed of properly and/or sink is drained
12. remove and dispose of gloves into waste container
13. wash hands
Count and record radial pulse - ANS 1. explain procedure, speak clearly, slowly, directly,
maintaining face-to-face contact whenever possible
2. place fingertips on thumb side of client's wrist to locate radial pulse
