Broad Spectrum Antibiotics - -used until the wound cultures have returned

-make sure to collect cultures before starting the antibiotics



broad spectrum antibiotics are avoided because - -they could lead to super infections or
SECONDARY infections and even sepsis.



Carbon monoxide poisoning - -normally oxygen binds with hemoglobin

-carbon monoxide travels much faster than O2 (200xs faster) therefore it gets to hemoglobin first and
binds (oxygen cant bind now)

-client becomes hypoxic and tx is O2 at 100%.

Grafting - -remove the burned dead tissue until healthy tissue is seen

-good skin is taken from a healthy donor site and place over the burned area

-the donor site is an open wound, so a transparent dressing is applied until bleeding stops

-Then the door site can be left open to air

-IF the client is well nourished, they can re-harvest from the same donor site q 12-14 day

-IF the skin graft should become blue or cool then that's a sign of poor circulation

-the PCP may take a tuberculin syringe and aspirate blood or exudate from under the graft

-The physician will sometimes order for roll sterile q tips over the graft with steady, gentle pressure from
the center of the graft out to the edge for proper adhesion

-Why? if air, blood, or excaudate is accumulating under the graft, the new graft will not adhere leading
to total or partial loss of the graft.



How do you apply topical agents to a burn? - Apply a thin layer of medication using sterile gloves
(sterile technique)



How long can silver dressings be left in place? - 3-14 days depending on the clients clinical
situation
