1.1 Morfologie van het bloed
1.1.1 Inleiding: samenstelling van bloed
Gemiddelde volwassene heeft ongeveer 5-6l circulerend bloed
(8% van totale lichaamsgewicht)
Samenstelling:
o Plasma
Extracellulair vocht met mineralen en eiwitten
Waterige oplossing met:
Elektrolyten
Plasma proteïnes
Koolhydraten
Lipiden
Normale proteïneconcentratie = 7g/dl colloïd osmotische druk = 25 mmHg
o Bloedcellen (in plasma gesuspenseerd)
Rode bloedcellen = erytrocyten
Volume totaal aantal bloedcellen uitgedrukt in hematocriet
o = maat voor concentratie rode bloedcellen
o Mensen met laag hematocriet bloedarmode = anemie
o Zeer hoog hematocriet = polycythemie visceus bloed
Bloedplaatjes = trombocyten
Witte bloedcellen = leukocyten
o 40-45% volume bloed bestaat uit cellen (meest rode)
Cellulaire componenten bloed onder microscoop:
o Erythrocyten

Mobiliteit hangt af van moleculair gewicht en elektrische lading

1.1.2.1 Morfologie van het beenmerg
Embryonale hematopoiëse :
o Eerst in dooierzak
o Nadien in fœtale lever en milt
Postnatale leven
o Bloedaanmaak in rode beenmerg in botten
Verschillende soorten:
allebei opgebouwd uit reticulair bindweefsel
o Rood, actief hematogeen beenmerg
Rood door veel erytrocyten en precursoren
Bevat nesten van voorstadia van bloedcellen
In mazen van reticulair bindweefsel = hematopoietische eilandjes
Veel macrofagen
Ook lymfocyten en plasmacellen
Aangevoerd vanuit secundaire lymfoide organen
Verblijven slechts transiënt in beenmerg
Grote hoeveelheid capillairen met endotheel bekleding
o Geel, inactief beenmerg
Rijk aan vetcellen
Kan in geval van verhoogde nood aan hematopoiëse terug overgaan naar
rood beenmerg innesteling van hematopoiëtische stamcellen vanuit bloed
Rood beenmerg gat in leven progressief over naar geel beenmerg
o Bij volwassenen enkel in axiale beenderen nog rood beenmerg

1.1.2.2 Bloedcelvorming in het beenmerg : hematopoiëse
Prille begin hematopoiëtische stamcel ontwikkelt in richting van bloedcel
o Stamcel vermenigvuldigt door celdeling of differentieert tot specifieke voorlopercel
o Afhankelijk van hormoonachtige stoffen groeit stamcel uit tot rode bloedcel, witte
bloedcel of bloedplaatje
Witte bloedcel onderverdeeld in lymfocyten, monocyten en granulocyten

1.1.3.1 Macroscopisch uitzicht
Milt heeft belangrijke functie in bloedaanmaak en afbraak + afweer tegen bacteriën
Macroscopisch:
o Boonvormig, puerperrod
o Gelegen in linker hypochonder
o Gemiddeld 12 cm lang, 7cm breed en 4cm dik
Oppervlakkig bedekt met viscerale peritoneum (sereuze bekleding) glad
2 polen
o Extrimitas anterior
o Extrimitas posterior
Binnenzijde facies visceralis
o Opgedeeld in 3 indeukingen:
Facies gastrica boven en voor hilus
Facies renalis onder en achter hilus
