D225 Field Experience Hours Log
Description: On my first field experience day I was able to experience how to access a patient port. The first patient had a diagnosis of breast cancer and was also seen for her venous access device. After accessing the port, we were able to utilize this to draw labs that were ordered for the day prior to cancer treatment scheduled for the next day. We then proceeded to review the next scheduled patient’s chart who has a diagnosis of iron deficient anemia to determine if she needed an Aranesp injection. The order parameters were to give if hemoglobin levels were
<11.0, the patient's Hemoglobin was 11.7 so the injection was not needed, and education was provided to the patient on not needing the injection this time. The next thing we completed was going through patients’ treatment plans for the next day's appointments and make sure the orders were all entered properly and printed
them off in the order of appointment time. Next, we were able to join in on a weekly
tumor meeting. This meeting goes through some tumor patient cases where it goes over the diagnosis, scan results, pathology, and recommended treatment outlines. After going through all this at the end everyone is given the opportunity to give any
other recommended treatment plans or agree with the current treatment regimen. Some of the examples included a patient with recurrent head and neck cancer the plan was to wait until the patient had signs of recurrence again before pursuing treatment. Another patient had Squamous cell carcinoma of the right ear, they had a Moh's surgery performed and then the treatment plan was to have several teeth extracted and then start chemotherapy and radiation treatment. There were a couple more patients that were discussed in this hour-long meeting. After the meeting was completed, we collaborated with the providers and pharmacy to make sure everyone knew what patients were on the list for appointments the next day, and what medications still needed to be supplied for treatments. Then we went through the charts of the patients for the next day to determine if any of their levels were severely abnormal that would prevent them or warrant a doctor's verification if the patient should receive their treatment or not. The last thing we did
at the end of the day was call the patients to confirm they would be coming for their
appointments that were scheduled for the next day. There was a total of 2 patients scheduled to be seen this day.

10/24/23: 8am-4pm
Description: Today I was able to access a port for a lab draw with the supervision of my RN supervisor. I was also able to start a maintenance fluid bolus through the port that this patient comes to two times a week for lab work and fluid boluses. Once the fluids were completed, I was able to de-
access the port. This patient has a diagnosis of Ovarian cancer and dehydration. For the next patient, I was able to access their port for a lab draw and administer a subcutaneous TBO medication. This patient has a
