Clinic Introduction
Currently in the local health system, MidMichigan Health, a barrier for consumer to
receive care is lack of access or transportation to care facilities (MidMichigan Health, n.d.-c).
Through interviews with nurse executives and articles published by MidMichigan Health, it is
reported that “no shows” and late cancels for appointments are one of the biggest losses of
revenue for the organization. This occurs not only for that specific appointment but for the
patient this could result in frequent readmissions for inpatient hospital stays. The proposed
service plan would be to develop a mobile chronic heart disease clinic. This clinic would travel
to the surrounding rural townships to hold clinics to provide basic cardiology care and services.
The plan would be to have “cohort” type sessions in which we would gather as a group and have
a class on various aspects of managing the disease and the cardiologist would follow with each
individual as well. This would benefit the patients and the community to by bringing the care
closer to them and removing a burden that often prevents them from receiving routine follow up.
Importance
In 2016, MidMichigan Health released their report on community health needs
throughout their region. It is reported in the Gladwin County Health Needs Assessment, that
6.4% of the population does not have reliable transportation and therefor makes it difficult for
them to travel outside of the immediate area. There is however a transit system that could be
utilized but only during traditional business hours (MidMichigan Health (n.d.-a). The same
report also states that 17.2% of the population has chronic heart disease and another 7.4% has
atrial fibrillation. In 2019, the update atrial fibrillation was reported at double the amount
