Identifying information:
Timothy Childers is a 15 year old Caucasian adolescent male. He is born and raised in
Minnesota. He is in the 10th grade of High school. He is currently in the school band and enjoys
hip-hop class after school. He has two older siblings, a brother who is 16 and a sister who is 20.
He lives with his brother, mother and father. His parents have been married for 23 years. They
have a 7 year old golden retriever dog. His older sister moved out of the house at the age of 16
when she got married. Timothy and his friends like to go to games, the movies and the mall,
when his parents allow him to. Timothy likes to watch his brother play football and basketball.
He has no interest in being on a team for football or basketball. He enjoys racing cars, going to
the mall, movies, and games when his parents let him. He likes to race cars. His career goals are
a NACCAR driver or a drummer in a band. He likes to talk to girls that he feels are “attractive”.
Girls think that he is cute and like him. Background information was obtained from his mother,
Timothy himself, educational and medical reports. This information was obtained from
interview, developmental history and rating scales as well as medical records. This information
appears to be from reliable sources and valid. Current position of Timothy’s learning and
behavior were acquired by observation during testing. Both from standardized psychological,
neuropsychological and achievement tests. Timothy’s performance on some of the tests were
found to be accurate with his cooperation and the motivation was also performed during the tests.
Reason for referral:
Timothy was referred for neuropsychological evaluation for possible learning disability and/or
Attention Deficit/Hyperactivity Disorder. Timothy feels that there is no need to be at this visit
and that his parent and teachers are the ones with the problem. As noted by his mother, timothy
has trouble with multistep tasks, grammar and vocabulary, and being able to organize his
homework assignments. Often times the teacher feels that timothy cheated on this math test
because he got the right answer but did not show his work.
Current Symptoms/Presenting Concerns
Timothy does struggle with his grades and academic performance. He can go from a B grade
student to a D student and then back to getting A’s. There is no family history of learning
disabilities or ADHA on either parent’s side. He does at times struggle with test taken if the
teachers do not follow the book. He may have some learning issues in grammar and vocabulary.
His ability to be able to maintain motivation in schoolwork is of concern. Recommendations will
be made with all these factors. Timothy’s clinical profile shows that there are signs of behavioral
problems, authority conflict, and family disorder. There are times where he may show signs of
being resentful, seek attention, and be moody. He may also appear to be argumentative and his
poor judgement at times can get him in trouble.
