INSUFICIENCIA RENAL AGUDA (IRA): Disminución rápida del filtrado glomerular (horas o dÃas) con
retención de desechos nitrogenados, perdida del equilibrio hidroelectrolÃtico y acido base.
ï‚· Objetivos:
- Limitar la acumulación de productos de desecho metabólico
- Mantener el equilibrio hidroelectrolÃtico
- Reducir al mÃnimo el catabolismo proteico, preservar masa muscular
- Mantener o mejorar estado nutricional
- Promover recuperación renal
ï‚· Estrategias (dependen del estado nutricional, grado de catabolismo, Ãndice filtración glomerular remanente,
indicación o no de tratamiento sustitutivo)
ï‚· Necesidades nutricionales:
 VCT: 30-40 kcal/kgp/d
 PR: 0,6 g/kgp/d principalmente AVB. Si estabiliza y mejora estado clÃnico 0,8-1g/kgp/d. si no estabiliza tto
renal sustitutivo
 HC y GR: se distribuye entre ambos resto VCT
 LÃquidos: se adecuara en función del balance hÃdrico que desee obtener

recuperación esperable en más de 2 semanas: tto renal sustitutivo: diálisis
