Case Study: Mr. C

This paper is a case study on Mr. C. It will discuss the clinical manifestations he is

experiencing. It will touch on potential health risks for his condition and discuss possible surgery

options. This paper will include five actual or potential problems for Mr. C with rationale. The

staging of end stage renal disease (ESRD) and considerable factors will also be discussed.

Patient education for Mr. C will be included, along with resources for ESRD followed by a

conclusion.

Clinical Manifestations of Mr. C.

Mr. C is seeking medical advice regarding possible bariatric surgery due to him being

obese. His current weight is 134.5 kg. Mr. C also has hypertension with a blood pressure of

172/98 at the time of assessment. He has 3+ pitting edema in his bilateral feet and ankles, a

fasting blood glucose of 146, an elevated total cholesterol level of 250, high BUN lab of 32,

elevated serum creatinine of 1.8, and elevated triglycerides of 312. He is also experiencing

tachypnea with a respiratory rate of 26 breaths per minute and has sleep apnea. Mr. C has some

complaints of pruritus and reports shortness of breath with activity.

Potential Health Risks for Obesity and Bariatric Surgery

Mr. C will need a consultation with a bariatric surgeon. They can work together to

determine whether or not the benefits of surgery outweigh the risk factors. Although Mr. C may

benefit from bariatric surgery, he also has a high operative risk that may cause complications

post surgery. “The main risk factors are age, male gender, body mass index, obstructive sleep

apnea syndrome, insulin resistance and diabetes, tobacco abuse, cardiovascular disease, ability to
