NR 443 Week 3 Graded Discussion Topic: Caring for Vulnerable Populations
Choose one of the three attached case studies, and reflect on the vulnerable population that the
case study portrays. Identify which case you chose. Discuss several risk factors that may impact
health outcomes for the vulnerable population in your case study.
Identify one agency in your community that can assist this vulnerable group and assess this
agency in terms of the 4 A's (see the lesson). Discuss each of the A's separately to assess
accessibility, acceptability, affordability, and availability for this population.
I choose the senior with the dementia case study for this week discussions. “The medical and
social needs of older adults present a unique challenge to health care providers. For example, the
prevalence of dementia is increasing, and issues such as abuse, and neglect of the elderly are
receiving much-needed attention,” (Dementia Society). Some of the risk factors that may impact
the health outcomes of Mr. Johnson are; impairment in activities of daily living (ADL),
malnutrition, lack of social network, and dependence on a care provider. People with dementia
can be extremely vulnerable due to the nature of their condition. Early symptoms can affect
communication and reasoning skills and consequently, they may not be able to understand or
explain to others what is happening to them. Simple tasks that can be done every day is
especially harder for seniors with dementia.
After reading the case study I think that Mr. Johnson is having caregiver role strain since
their grown children are out of State and she might not have anybody to help with the care of,
Mr. Johnson. Nurses can play a significant part in reducing the role strain caregivers feel or
preventing its occurrence. “Caregivers often become victims of their own compassion when
caring for loved ones who can no longer look after themselves. Because caregiving can be an all-
encompassing, 24/7 responsibility, caregivers can easily lose sight of their needs and risk their
own good health. This is especially true if the person being cared for suffers from a long-term
progressive disease such as dementia or Alzheimer’s disease.” “Community health nurses must
advocate for vulnerable groups and provide sensitive care to empower those experiencing
inequities.” (CCN,2018) One agency in my community that can assist Mr. Johnson is New Path
Home Health Care.
“Community health nurses evaluate available resources using the four A's, which helps to
assure that the resource is meeting the needs of those in the community” (CCN,2018). One
agency in my community that can assist seniors with dementia is New Path Home Health Care:
Accessibility: New Path Home Health Care provides compassionate, patient, caring and
professional service to seniors in the comfort of their own homes.
Acceptability: According to the case study this place would assist Mr. Johnson perfectly
because it offers nursing supervision 24/7 a week to assist patients with their medicines
and all other medical necessities. Also, it has the personnel to assist patients with
therapies and daily living activities.
Affordability: New Path Home Health Care offers a free consultation and their rate are
very affordable. First time client will receive a discount. This Agency also accepts all
types of insurances.
Availability: This center offers is services at the center or in the home according to the
patient’s preferences. It has a specialized program for the patient with Alzheimer’s and
Dementia.
