Deming’s principle of continuously evaluating the systems and process to improve and deliver high
quality cost efficient care to our patients at St. Louis Medical Center is a principle the nursing
administration implements to achieve goals. Readmission continues to be a struggle for all healthcare
facilities especially the elderly populations. Over the last several months, SLMC has experienced an
increase in elderly patients readmitted into their hospital for congestive heart failure. Because of this,
SLMC has revamped their patient-centered goals, objectives, and increase training for nurses. St. Louis
Medical Center has improved significantly with their delivery of safe, effective high quality of care. The
improvements made by this organization continues to decrease the number of readmissions, but it has
not stopped them. The nursing administration must explore the reason behind the readmissions by
auditing the patient’s medical records to gain insight on areas to improve for prevention of readmits in
the elderly population. Hospitals are penalized by reduction of payments from the Center for Medicaid
and Medicare services by the amount of readmission within thirty days and the quality of care received.
The financial penalty is for patients diagnosed with six medical conditions is congestive heart failure,
myocardial infarction, pneumonia, chronic obstructive pulmonary disease, total hip, and total knee
arthroplasty (Banerjee, Thompson, Kell, Shetty, Vetteth, Grossman, Fowler, 2017).
Internal audits are essential for growth and development within healthcare organizations as the audits
provide insight into prevention and risk factors for future references (Lear, 2016). The internal auditing
process will consist of a team of auditors that will investigate the readmission of congestive heart failure
patients once the patients are discharged each month. Any nurse can volunteer to be an auditor. The
tool would include data on an excel spreadsheet collected from the patient medical history, the number
of admissions, physicians, mental status, and level of education, sensory issues, nutritional intake, living
arrangements, discharge home or facility, callbacks data, current medications, and financial situation. We
will examine the patient’s risk factors and comorbidities that contribute to their readmissions. Discharge
callbacks for post-follow-ups are conducted to evaluate the patient knowledge and understanding of the
discharge instruction, medications, and follow-up appointments. The information obtains during the
callbacks determine if the patients a high risk for readmission and schedule a post-discharge education
follow-up. The data obtained from the audits are reviewed on the excel spreadsheet. The auditors can
make a bar graph showing the trends of what is working and areas of improvement needed to decrease
admits in this patient populations. The team can develop action plans to implement to increase
compliance, reduce admission, and reeducate nurses about heart failure. The audits are focused on
increasing the patients knowledge regarding their medical condition and improving their outcomes.
