COMMUNITY ASSESSMENT, ANALYSIS, and NURSING INTERVENTION
Modules 1-3 (Weeks 1 – 3) Assignment Template
Use of the Three-Part Course Template
● The use of the three-part template to submit weekly Major Course Paper Assignments goes as
follows:
● Week 1: Windshield Survey and Key Informant Interviews. Complete the First Section
of the course template and submit. (Keep this section attached to Weeks 2 and 3 assignments.)
● Week 2: Statistical Analysis of Community Health Problem. Complete the Second Section
of the course template and submit. (Keep this section attached to Weeks 1 and 3 assignments.)

● Week 3: Community Health Nursing Intervention. Complete the Third Section of the
course template and submit. By now, you should have Sections 1-3 as one whole document.

● Each section of the course template is submitted on a WEEKLY basis, depending on which week you
are working on. DO NOT wait to submit all three sections at the end of the third week.

● Please note that students are not to make any changes to the content of the course template, for
example, removal of questions or examples imbedded in the response box of the template.

● Removal of the questions or examples imbedded in the response box of the template is
considered tampering with the content and will result in a 10-point grade deduction per
assignment.
Should removal of the questions or examples imbedded in the response box of the template persist by the
student, in addition to the 10-point deduction, the student will be referred to the Office of Student
Conduct for final disposition. You are strongly encouraged to be mindful of this course policy.

● The ONLY change students may make to the template is to enlarge the space to make their entries in
the response boxes.

● Should you encounter any difficulties with these course policy guidelines, please consult with your
Academic Coach.

Elements Descriptors
Physical Make sure you address fully what you observed and NOTE if
Environment something was not found within your community area.

● Boundaries What defines the boundaries? Roads, water, railroads. Is the area
known for a particular name? What area is this community located in?

This is located in Canyon Lake, Texas. It is bordered by 281 North to the
east, FM 3009 to the north, FM 2673 to the south, and IH-35 North to the
west. It’s about 30 miles northeast of San Antonio.

● Housing and What is the age of the houses? Describe type of construction material,
Zoning appearance, and general condition. Do you see single family housing?
Multi-family housing?

The houses vary in age, depending on where you are around Canyon Lake.
There are much newer houses on the northern shore, and older houses in
Sattler (on the south side of the lake). Construction material and types range
from pre-fabricated homes to multi-million dollar homes on hilltops, which is
vastly predominated by single-family housing. There are little to no multi-
family housing or apartment complexes.

● Common Where do people in the neighborhood hang out? Who hangs out there

, Meeting Places and what hours of the day?

There are several places to hang out. Two common ones are the VFW
buildings, of which there are two: one in Startzville and one in Sattler. These
were built to attract retired veterans, but anyone can go. The Guadalupe
River and Canyon Lake are here, which often draw younger crowds during
the warmer months.

● Indicators of Is this a “thriving” community or does it look “run down?” Is there new
Growth or Decline or ongoing construction? Businesses and Industries coming to the
area? Are buildings, roadways, and public areas clean and well-kept.
Do you see dirt, trash, or graffiti?

While this area is quite rural, it’s also growing quickly. 30 years ago, it used
to be only known as a hangout for high school and college kids who wanted
to get away from Austin and San Antonio, but it’s slowly started to become
more domestic in setting. Many of the businesses in the area have difficulty
staying open for more than 6 months, but this is other reasons (like people not
thinking before they open an establishment). The area is dominated by farm-
to-market roads, which are pretty well maintained, with minimal dirt, trash, or
graffiti.

● Employment What places for employment are there is this area? Name the
Opportunities businesses and industries in this area.

There are several schools within a few miles. There’s a hospital called
Resolute Hospital. There’s the Schliterrbahn waterpark, the largest water
park in Texas. Lastly, there’s GVTC (the Guadalupe Valley Telephone
Company).

● Transportation How to people get from one place to another? Is public transportation
available? How timely is it? Personal autos? Bikes? Walking?

Most everyone uses personal vehicles for transportation. While there are
LYFTS and Ubers that occasionally come through, these are more expensive
options, since they usually have to drive from San Antonio or San Marcos.
There are no trains or buses.

People Make sure you address fully what you observed and NOTE if
something was not found within your community area.
