A. Understanding of Nursing Quality Indicators
The scenario that is being analyzed has many points that are troubling and easy to prevent
if assessed and acted on quickly by the Registered Nurse. The first point would be the “red
depressed area, over the lower spine”. This is a hallmark of a larger issue to come, pressure
ulcer. Mr. J. is a 72 year old patient recovering from a fractured right hip. Dementia which
elicits confusion when accompanied with pain medication has caused him to be restrained.
These restraints have limited his movement, causing him to lay in one position for an extended
amount of time which has caused the bony processes of his lower back to compress the skin and
slow blood flow causing redness and damage to skin. A nursing quality indicator for pressure
ulcers would be very important in determining the type of pressure ulcer and direct the correct
care for the patient. Research has found that 1 in 3 hospital nurses only sometimes felt confident
in their ability to stage a pressure ulcer (Berquist-Beringer 2009). A pressure ulcer education
program would standardize the staging of a pressure ulcers with the use of pictures. Once the
Registered Nurse is able to correctly stage the ulcer then treatment plans and interventions can be
performed. In the scenario, the CNA should report what was seen and stated by the family
member. This would have caused the Registered Nurse to assess the pressure ulcer and report to
the family member that repositioning of the patient will decrease the redness and return natural
color to the area.
The second point within the scenario is the use of restraints. Restraints are highly
effective at keeping confused patients within their bed which can aid in reducing the chance of a
fall or other injury. Restraints should never be used as a primary or first treatment for a confused
patient, they should be last resort. Restraints limit the mobility of the patient and in this scenario
