Excess consumption of meals and snacks although father stresses the importance of a
well-balanced diet
Active in physical activity weight lifting or football at least 1 hour per day. Allowed to
watch 2 hours of TV per day.
Family History: Parents alive. Father suffers from obesity, HTN and Diabetes Type 2, father
had Gastric Bypass 1 year ago. Mother no known history. Both Paternal and Maternal
Grandparents are alive, MGM and MGF no known history. PGF- obesity, HTN and Diabetes
Type 2. PGM- no known history.
Social History: patient currently in 11th grade this school year. He is active and plays football
and, on the weight, lifting team for his school. Father states he or and the mother do not smoke.
They drink on occasion. Patient does not drink alcohol, smoke or use illicit drugs. Allowed to
watch TV 2 hours per day.
ROS:
Constitutional Symptoms- reports minimal pain to left foot, able to bear weight but
uncomfortable. Denies any bleeding, popping or cracking to foot.
EYES - denies any blurred vision, difficulty focusing or any visual disturbances
Ears, nose and throat- patient denies any difficulty with hearing, denies any sinus
problems, nose bleeds or nasal congestion, headaches, dizziness, sinus problems, no
difficulty swallowing or pain
Cardiovascular: father reports patient is active and participates in sports. He participates
in some sort of physical activity at least one hour each day. Denies any history of heart
murmur, chest pain, palpations, dyspnea, activity intolerance.
Respiratory- denies any history of respiratory issues
Musculoskeletal- reports minimal pain 3/10 to left foot, with swelling. Denies any back,
knee or ankle pain bilaterally. Minimal difficulty in ROM to left foot able to move and
ambulate with pain
Integumentary- reports edema and bruising to dorsal of left foot, denies any nail
deformity, urticaria, or other skin changes
Neurologic: denies headaches, weakness, numbness, tingling, memory difficulties,
involuntary movements, syncope, seizures or paresthesia’s
Psychiatric- father denies mood changes, anxiety, depression, insomnia, suicidal
thoughts, exposure to violence or excessive anger
Hematologic/lymphatic- denies any unusual bleeding or bruising, lymph node
enlargement or tenderness, fatigue, no history of anemia or blood transfusions
Allergic/immunologic- patient denies seasonal allergies or allergy testing, exposure to
blood or body fluids, use of steroids or immunosuppression in self or family
