1. Explain the purposes of providing hygiene care and how to assess for hygiene

● Purpose: clean skin, decrease microbial load, increase circulation, self image and well-
being, reduce body odors, promote ROM passive or active, great opp for assessment,

● Difficulties: Assess for difficulties during nursing hx or physical assessment
○ Nursing hx- determine self care practices and hygiene abilities, past or current
probs, ID of pt’s at risk of developing impairment
○ Phys assess- need for hygiene, skin or other impairment
● Nursing diagnosis ex- self care deficit, impaired skin integrity, altered oral mucous
membrane, risk for injury or infection, situational low self-esteem, deficient knowledge,
disturbed body image
● Nursing planning ex- when appropriate the pt and/or fam sets goals/outcomes
○ Nurse ID interventions to assist client to achieve outcomes
● Nursing interventions ex- assist dependent pt w hygiene, educate pt and/or fam about
appropriate hygiene practices, demonstrate use of assistive equipment and adaptive
activities, assess and monitor physical and psych responses
● Nursing eval ex- did we achieve our goals and interventions? Do we need to revise the
plan?
2. Discuss delegating hygiene care to other health care personnel.
● Assess situation prior to delegating for limitations, pt needs. Instruct nursing assistant
about limitation and needs. Do they need a cane? One person assist? Total care pt?
● Give instruction about things they may see to report back to you, the RN
● Delegating can be necessary. CNA’s are equipped to give baths and perform other
hygiene tasks. It is important for Nurses to also perform these tasks in order to do skin
checks and build rapport with patients as well as co-staff. Need to give them pertinent
info about patient: total care, ambulatory, tubes, IV’s, etc.
3. Discuss factors that influence personal hygiene practices.
● Age (don’t need to bathe as much as older adult), culture, religion, developmental level,
financial status, health & energy level, preferences, advertising, access to running water
4. Discuss adaptations to personal care based on special patient populations (ex:
diabetic, unconscious, etc.)
