Amira is a 27-year-old Syrian refugee who has been residing in a local homeless shelter
since her arrival here in the United States 4 weeks ago. She was brought into the
emergency room this morning via squad after being found by a shelter employee sitting
in a pool of blood on the bathroom floor crying and holding her abdomen. Due to her
limited English speaking abilities, she is unable to provide specific details as to her
complaints but the shelter employee states that she has recently stopped eating and
has not looked well for the past couple of days.
Based on the limited information provided, please answer the following questions.
1. How will you prioritize your care of Amira, what assessments will you complete,
and in what order? Please provide rationale for choosing this order.
2. Are there any cultural beliefs/practices that must be taken into consideration
when planning her care?
3. Considering her symptoms of abdominal pain and bleeding, is it possible that her
status as a homeless refugee is a causative or contributing factor to her illness?

As a nurse taking care of Amira my primary concern is to find the source of her
bleeding. There are many factors that I need to assess and I need to gather verbal
information in order to complete a good assessment. There is a language barrier
between Amira and I; therefore, I need to get translating services involved immediately
so that Amira and I can communicate. “A language barrier has been shown to be a

As part of my assessment, I would focus mainly in her abdominal and genitourinary
area to determine the source of her bleeding and pain. I would auscultate her abdomen,
listening for bowel sounds and vascular sounds or bruits. Palpating her abdomen to
determine what area is the pain located at. I would recommend to the doctor that an
ultrasound should be a priority. The ultrasound type I would recommend would be a
Transvaginal ultrasound. “Transvaginal ultrasound is a test to look at a woman’s uterus,

Part of the stat lab work I would suggest would be a CBC, BMP, PT, PTT, Type and
Screen, HCG in serum and in urine (urine results are obtain faster).
Assessing Amira’s circulatory status is also a priority due to the loss of blood. Blood loss
symptoms are cold or clammy skin, pale skin, rapid shallow breathing, rapid heart rate,
