Theoretical Basis for Advanced Nursing-Practice

Introduction:

A concept represents the building block of theory. It is the core of what researchers want

to study. A concept analysis can clarify the meaning of a concept these days which can help in

the development of the concept in the future. Within this concept analysis, there will be a

discussion on the concept of compassion fatigue in nursing. I will provide in this concept

analysis the definition of compassion fatigue in nursing, defining attributes of the concept

discussed, antecedent and consequence of the concept, a model case, theoretical application of

the compassion fatigue concept, and how compassion fatigue is applied to advanced nursing.

Definition/Explanation of the selected nursing concept:

Concepts are valued as the elements of theory. Research in nursing is based on evidence-

based practice and theory and they both rely on a clear understanding of what is being studied

which is the concept. Concept analysis is the study of a concept; it makes it easier to understand

the concept. In 1994, the Walker and Avant method of concept analysis was introduced and was

frequently used in nursing and it has been used in the analysis of a wide range of concepts (Butts

& Rich, 2018). The concept of compassion fatigue was first introduced in the early 1990s in

North American and was used to explain a phenomenon observed in nurses employed in the

emergency department (Crewe, 2016). Compassion fatigue or what is known as “burnout” is

commonly identified as “a syndrome of emotional exhaustion, depersonalization, and reduced

, accomplishments that occur among individuals who do people work of some kind” (Sabo, 2011).

According to Cross (2019), an initial compassion fatigue concept analysis was considered as a

secondary traumatic stress disorder. Through the years, more literature was conducted to provide

clarification of compassion fatigue and to assist in future recognition in the nursing profession

(Cross, 2019).


Defining attributes:

Nurses face challenges on daily basis and find it hard to maintain the balance between

work stressors and everyday stressors. Nursing is a demanding profession and most nurses in the

field are in it to help others and provide empathetic care for their patients. Compassion fatigue is

considered the exhaustion of physical and emotional care for patients due to prolonged exposure

to trauma from treating patients who have directly experienced traumatic and stressful events

(Kelly & Lefton, 2017). According to Walker and Avant, attributes are the characteristics that

are often associated with the concept. Multiple essential attributes define the concept of

compassion fatigue. Three attributes of the concept of compassion fatigue are psychological

stress,

witnessing negative experiences of others, and depression.

Nurses who experience psychological stress daily are more likely to develop compassion

fatigue. Nursing is high demanding job and nurses face daily challenges in their workplace.

Stressors that contribute to psychological stress in nursing are related to the excessive workload

due to lack of staff and poor leadership and management. Lack of support from other coworkers

and work management can put a toll on the nurse. The high nurse-to-patient ratio is a rising

problem and can lead to nurse burnout and compassion fatigue. Other triggers for psychological

stress among nurses include harassment and workplace violence (Chachula, 2020). According to
