A1. Three quality improvement initiatives at Fielder Medical Center are Central Line-
(CAUTI), and a prevention program for readmissions. Why the initiatives are necessary to the
center are as follows:
First, Central Line-Associated Blood Stream Infections (CLABSI) are central line
catheters inserted via the chest, neck, or groin to provide fluids, give medications, or collect
blood. The catheters can remain intact for weeks or months at a time. The prolonged period that
the catheter can stay in the human body can cause infections, such as bloodstream infections. A
bloodstream infection occurs when bacteria travels through the central line. To decrease the rate
of infections, Fielder Medical Center established a team to research ways to reduce these
infections in the most cost-effective manner and created the program called "stop the line." Staff
was trained on how to insert a central line catheter properly/how to continue upkeep while the
line is in place. After 18 months, it was remarkable that infections dropped almost 50%, bringing
their rates below the national average.
Second, Catheter-Associated Urinary Tract Infections (CAUTI) is a urinary tract infection
(UTI) caused by a urinary catheter where bacteria/viruses travel through the catheter and into the
bladder/kidney. This can result in being a long-term issue, such as when recovering from surgery
or bedbound. When not treated properly, the individual will experience unpleasant symptoms,
excessive use of antibiotics, or possible sepsis, which is the body's natural response that can shut
down organs. Therefore, proper care/maintenance of urinary catheters is very critical to reduce
, rates of CAUTI. In response to this, Fielder Medical Center compiled a collaboration of
healthcare providers and caregivers to identify/implement the best way to prevent infections in
the hospital. They collaborated with other hospitals to gain information on ways to avoid the
issue. The team trained staff on supplementary uses besides urinary catheters and educated
physicians and nurses to remove catheters promptly. Improvements were made in the
care/upkeep of urinary catheters, and Fielder Medical Center observed these efforts were
working as their number of CAUTI infections decreased.
Third, to prevent readmission to the hospital, Fielder Medical Center started a program to
ensure smooth transitions and experiences were taking place upon discharge to either the
patient’s home or another facility. Per nation data, one in five Medicare patients will end up
readmitted within 30 days of discharge. Readmissions trigger stress and financial stress on
patients. Therefore, the Fielder Medical Center executed a program to flag high-risk patients who
are prone to return, such as those with congestive heart failure/pneumonia patients. All
healthcare personnel involved in a patient’s discharge now take extra steps upon release to
educate the patient/family, such as future care or any side effects of medications.

their internal organizational processes. The Fielder Medical Center possesses quality
improvement projects that comprise of measurement (how are they doing), assess (meeting
expectations), and improve (how to improve performance can occur) patient care. Here we are
