A1. Access
Japan’s health insurance system provides universal coverage to its citizens (Japan, 2020).
It is funded mainly by individual contributions and taxes and enforced by the government.
Everyone is covered and can participate with the plans; citizens enroll in either an employment
or a resident health insurance plan. Plans are provided by employers are mandatory paid by both
employer and employee. Most citizens have private health insurance, but life insurance can be
used coinciding with them to provide additional income if needed for an illness. Citizens are
expected to pay 30 percent of services and some copays. Young children and low-income elderly
adults have lower rates (Japan, 2020).
Older adults, low income, and children are required to pay an annual household fee set by
the government (Japan, 2020). This covers their health care and long-term services; the fee is
established based on their age and income. Non-citizens who have lived in Japan after one year,
are able to get free access to healthcare, but undocumented immigrants and visitors are not
offered health insurance (Japan, 2020).
The United States insurance system does not provide universal coverage to its citizens
(United States, 2020). Private insurance is provided primarily by employers, and most provide
several choices of insurance plans. Most plans cover the employee and their dependents, and
