Informatics has assisted the healthcare environment with bridging the gap between the
ever-growing industry of technology and the usability of electronic “tools” that better patient
care and access to information. Everyday we are evolving, and the way we practice medicine
should too. It is important to push our abilities to better inform, educate, and treat our patients
through the use of technology. Through this week’s discussion board, I came across another
student’s description of a program used in their hospital that I believe could do wonders in my
healthcare facility as well, and for that reason I am choosing to incorporate that idea into my
proposal and expand upon it further. The following paper will describe the patient outcome
improvement proposal and how it can be incorporated, the projected benefit to patients, the
stakeholders involved, and the team needed to make it a reality.
The Proposed Project
As nurses, we are tasked with a considerable work load in the course of 12-hours.
Between assessments, charting, medication administration, patient care, and fulfilling physician
orders – we are also constantly monitoring our patient’s vitals, labs, and clinical status so that we
are ready to intervene and treat as their condition changes. That is a lot to manage, but of all the
items just mentioned, note that I didn’t talk about patient education. This idea to most nurses
comes as a given, and is something that happens on a continuum throughout the patients stay in
the hospital. Every encounter presents itself as an opportunity to teach the patient about their
plan of care, goals of care, medications, and interventions. But let’s stop and think about it from
, the patient’s perspective for a moment. These individuals are sick, tired, out of their
environment, and let’s be honest – overwhelmed. They aren’t medical professionals and more
often than not they don’t understand all of the medical jargon that is being thrown their way by
the various doctors, specialists, nurses, and other staff that come into their room sporadically
through the day. They want to ask questions, but often times they can’t think of what to ask until
the doctor has already left the room. In the healthcare system I work for – University of
Pittsburgh Medical Center (UPMC) – there is currently a system that is used to help keep the
patients informed of their care each day, it is called the “Daily Plan of Care”. This comes in the
form of a paper packet that is printed out each day at 10:00 am and distributed to the patient. In
this packet is the names of each member of the patients care team, their current medications,
their most recent laboratory results, and any tests or procedures that are scheduled for the day.
This quality improvement project has been in place for approximately 5 years and seems to help
patients feel more involved in their care, and allows them a place to write notes or questions so
that they may be better prepared when their physicians round. But what if we used technology to
help us make this idea interactive and educational? There are other facilities using patient
engagement software that provides all of the above information to patients, but does so in
electronic form on either a tablet placed at the bedside, or the television mounted in the room.
One such company that I discovered is called the GetWell Network. Their software allows a
fully interactive portal for patients to be engaged and active in their care. Via touchscreen, the
patient can select a multitude of options including their medication list, watch educational videos
that are tailored to their diagnosis and treatment plan, select their meals for the day, even see
what is planned for their day or review their test results (GetWellNetwork, n.d.). The use of this
interactive forum has shortened the length of inpatient stay by 20.5%, and decreased heart failure
readmission rates by 35.6% (GetWellNetwork, n.d.). I propose that UPMC Network incorporate
this software into their operating system across their 40 hospitals nationwide. Rather than hire a
company such as the GetWell Network, I propose that UPMC develop their own version of this
