A forty-nine-year-old patient with rheumatoid arthritis comes into the clinic with a

chief complaint of a fever. The patient's current medications include atorvastatin 40mg at

night, methotrexate 10mg by mouth every Friday morning, and prednisone 5 mg by mouth

every morning. He states that he has had a fever up to 101 degrees Fahrenheit for about a

week and admits to chills and sweats. He says he has had more fatigue than usual and

reports some chest pain associated with coughing. He admits to having occasional episodes

of hemoptysis. He

works as a grain inspector at a large farm cooperative. After an extensive workup, the patient

was diagnosed with invasive aspergillosis.

, Rheumatoid arthritis is a chronic autoimmune systemic inflammatory disease, that

mostly affects joints but may involve many tissues and organs (McCance & Huether, 2019).

RA is the inflammatory destruction of the synovial membrane, joint capsules, ligaments,

tendons, and



articular cartilage, with several clinical manifestations affecting the skin, lungs, heart,

kidneys, spleen, small and large arteries, as well as joints (McCance & Huether, 2019).


Invasive aspergillosis is a fungal infection with systemic characteristics, caused

mainly from exposure to aspergillus spp. and affecting immunocompromised patients

receiving immunocompromised agents (AL-Janabi, 2020).


Symptoms Presented


The chief complaint in this case study is a fever of 101 for about a week, which can be

a pretty common symptom of rheumatoid arthritis. Rheumatoid arthritis is an autoimmune

condition that causes inflammation; when cellular protein is recognized as foreign by immune

cells T lymphocytes, a cascade of destructive inflammation is set in place (Smith, 2020). The

fact that rheumatoid arthritis is an autoimmune condition and causes inflammation patients are

prone to fever, also long-term use of immunosuppressant agents can contribute to elevated

body temperatures. The sweats are caused by the rapid change in the body's core temperature

and

chills are the result of exaggeration of the normal circadian temperature rhythm. Fatigue is

very common in rheumatoid arthritis. Pain alone can cause fatigue, but fatigue generally

come from the increased effort with activity due to muscle weakness from lack of energy

and activity. Fatigue is also a side effect of prednisone and methotrexate. Invasive

pulmonary aspergillosis grows spores in the alveoli causing obstruction and producing

symptoms of chest pain with coughing and occasional hemoptysis (AL-Janabi, 2020).
