 Mania: abnormally and persistently:
o Elevated mood-euphoria or elation
o Expansive mood-lack of restraints in expression; overvalued self-
importance
o Irritable mood-easily annoyed and provoked to anger
 Manic episode: distinct period of mania
 Mood lability: rapid shifts in mood with little or no change in external events
 Grandiosity: exaggerating personal importance
 Rapid cycling: extreme form of mood lability that can occur in bipolar disorders
o in most severe form, it includes continuous cycling between subthreshold
mania and depression or hypomania and depression
 hypomania: overactive energy, mood, behavior, and activity levels
Bipolar disorders
 Mania or hypomania alternating with depression
o Bipolar 1 (major depressive, manic, or mixed episode)
o Bipolar 2 (major depression and hypomania)
o Cyclothymic disorder (hypomania and depressive episodes not meeting
full criteria for major depressive episode)
Mania
 Need for sleep is decreased and energy is increased
 Initially there is an increase in goal directed activity that is purposeful (ex:
cleaning the house), but it deteriorates into hyperactivity, agitation, and
disorganized behavior
 Can be excessive involvement in pleasurable activities with little regard for
painful consequences (ex: excess spending, sexual behavior, drug/alcohol
abuse)
 Risk taking behaviors
Bipolar 1 diagnostic criteria
 At least one manic or mixed episode and a depressive episode must occur
o Mixed episode: when mania and depression occur at the same time which
leads to extreme anxiety, agitation, and irritability
Bipolar 1 across the life span
 Children and teens
o Depression usually occurs first; marked by intense rage
o Symptoms reflective of developmental level of child
o Often have other disorders like ADHD or conduct disorder
 Elderly
o Greater neurologic abnormalities and cognitive disturbances
o Incidence of mania decreases with age
Bipolar 1 risk factors
 Fam hx of mood disorders, prior mood episodes, lack of social support, stressful
life events, substance use, medical probs (chronic/terminal illnesses)
