Addison's Disease - Answer - Primary adrenal insufficiency is dysfunction of adrenal cortices.
- Secondary adrenal insufficiency is result of insufficient ACTH.
S&S: weakness, fatigue, HA, nausea, hyperpigmented skin (lack of cortisol), sparse axillary hair, abd pain.
Diagnose: Abd CT, head CT, Cortrosyn ACTH stim test, pan culture.
Findings: low Na, hyperK, low glucose, elevated ESR, neutropenia, esosiniphilia, lymphocytosis, hyperCal,
low plasma cortisol, elevated BUN, metabolic acidosis.
Treat: NS or D5NS, IV hydrocortisone, electrolyte replacement, monitor for decomp., IV abx if indicated.
Cushing's Syndrome - Answer - Hypercortisolemia (iatrogenic from glucocorticoid use, pituitary tumors,
and ectopic ACTH-secretion from tumors outside of the pituitary).
S&S: round face, obesity/weight gain, hirsutism, HTN, lethargy, menstrual changes, abnormal glucose
tolerance, decreased libido, striae, acne, EKG abnormalities, osteopenia, female balding.
Diagnose: ask about steroid-use, 24-hour urine free cortisol test, overnight DST test, and late-night
salivary cortisol test.
Cushing's Tests - Answer 24-hour urine:
- measures cortisol secretion that is not bound to cortisol-binding globulin.
- needs to be done at least twice.
-Assay levels between 3.5 and 45 are NORMAL
