needle position, mixing tubes too vigorously, non sterile site prep, prolonged tourniquet
application, wrong collection time etc.

transportation of dangerous goods act (TDG) - Correct Answer federal legislation that
is in place to ensure the safety of all individuals handling the specimen as well as to
ensure the public's safety in the event of an accidental spill or leak. The act pertains to
the transport of all goods that are considered dangerous.

categories of TDG - Correct Answer Class 1: Explosives
Class 2: Gases (Dry Ice)
Class 3: Flammable and combustible liquids
Class 4: Flammable solids
Class 5: Oxidizing substances

Class 6: Poisonous (toxic) and infectious substances (all lab specimens)
Division 1: Poisonous Substances

, Division 2: Infectious Substances
Category A: Infectious Substance, Affecting Humans
Category B : Biological Substance, Including Waste

Class 7: Radioactive materials
Class 8: Corrosives
Class 9: Miscellaneous products (Formalin

TDG requirements for packaging - Correct Answer packaging of specimens for
transport to another laboratory include the following guidelines:
1. Place the specimen in a watertight receptacle. This inner container = primary
receptacle. Acceptable types include glass vacutainers (except if being shipped with
liquid nitrogen), screw-cap plastic tubes, or specially designed plastic bags for holding a
volume of formalin and tissue. Many laboratories further seal the vacutainer tube or
plastic screw-cap tube by wrapping the cap with a stretchable waxy paper
(commercially available as Parafilm®) or adhesive tape.

2. place the primary receptacle into a secondary receptacle, must be watertight. An
example of a secondary receptacle suitable for a single primary receptacle is a small,
cylindrical, metal container just large enough to contain the primary receptacle and
absorbent material. The primary and secondary receptacles are referred to as inner
packaging.

3. Place the secondary receptacle containing the primary receptacle(s) inside an outer
package that is durable and resistant to moisture, pressure, and possible temperature
changes. Many laboratories use durable plastic coolers (similar to the ones you may
take camping) with latch mechanisms on the lids. Other laboratories use Styrofoam-type
coolers inside corrugated cardboard containers. Wood or metal containers are
acceptable as well.

4. If the specimens are to be shipped refrigerated or frozen, extra preparations must be
made to ensure that they retain the appropriate temperature. Wet ice, pre-frozen packs,
or dry ice can be used to maintain refrigerated or frozen specimens. Place the coolant
outside the secondary packaging. The secondary packaging must be securely fastened
or supported inside the outer packaging so that if the ice should dissipate during
shipping, the secondary packaging remains upright and stable.

labeling requirements for TDG - Correct Answer Place an itemized list of the contents
of each package between the secondary packaging and the outer packaging when
shipping specimens.

Clearly label the outside of all packages containing specimens with the address and
telephone number of the consignee (the destination of a package). Also, label the outer
package to include:
• The general name for the contents (for example, infectious substances) and the TDG
classification (for example, most specimens are classified as Class 6.2).
