DD Form 1348 - -This form is used to issue out ammunition to Range OIC's.
Is also used when doing a turn in at the ASP, and must be created prior to
getting there. The ASP gives us one when signing for ammunition as well.

-DD Form 1907- Signature and Tally Record - -The only people allowed to
sign this form are those on the delegation of authority and must be AA&E
screened before going on the DOA.

-DD Form 626-Motor Vehicle Inspection - -Vehicle inspections will be
conducted before leaving the motor pool to the ASP (Ammunition Supply
Point) with the form. If there are any deficiencies they will be corrected
before leaving to the ASP.

-E581 Form - -This form is created through inputting the request on TAMIS,
it is also what will be needed in order to pick up ammunition at the ASP when
you get there. (Must have the E581 version that is approved by the ASP
itself.)

-If the BC cannot sign the DOA, who will sign for it? - -Executive Officer

-MDAS Form (Materials Documented As Safe) - -Dunnage is to be
certified/verified before leaving the range is or else it is still considered
MPPEH (Materials Potentially Pertaining an Explosive Hazard). The document
MUST TRAVEL with said dunnage to the location it will be SECURED and KEPT
AWAY FROM THE ENVIRONMENTAL ELEMENTS to i.e. Supply cage or
Quadcon.

-NAVMC 11381-Expenditure Report - -The form is partially filled out once
the person has the documents, after the range is over the expended section
is filled out and if there's a turn in then that portion is filled out after turn-in
is complete.

-NAVMC 11797-Delegation of Authority (DOA) - -Battalion Commander gives
us the authority to sign/request for ammunition.

-Who on the (DOA) needs to be (AA&E) Screened? - -Requestors, Receivers,
and CAT 1 SNCO's & Officers
