,Introduction
Promoting self-management is a module that focuses on promoting the patient's self- and co-
sufficiency. Based on Mr. X's case study, we look at where his self- and co-sufficiency can be
improved. This is done by means of a family-oriented assessment. Based on the results and
conclusions, a family meeting is organized to discuss this with the patient and his family.
After the family interview, the patient's self- and co-sufficiency is assessed and what
recommendations can be made. The recommendations are presented to Mr. using a
therapeutic letter.
This report focuses on a number of CanMEDS roles: caregiver, communicator, collaborative
partner and health advocate. Each role highlights specific competencies that healthcare

, professionals must possess to successfully carry out their responsibilities and provide
comprehensive care to patients (Enurse.nl, 2023).
The learning outcomes for promoting self-management are:
- describes the relationship between the concepts of self- and co-sufficiency, describes
how these concepts are applied in nursing and multidisciplinary practice, and
describes her own views regarding these concepts.
- performs a (family) system-oriented assessment in conversation with a user and one
or more family members using a validated methodology and appropriate
communication skills.
- conducts a (family) system-oriented interview with a care recipient and one or more
family members based on a validated methodology and appropriate communicative
skills.
- analyzes the care situation partly on the basis of the family or system-oriented
assessment and the conversation with care recipient and family.
- analyzes on the basis of EBP the possibilities of information and communication
technologies aimed at self- and co-reliance and keeps a critical eye on the use of the
chosen technology within the care situation concerned.
- formulates, in the language of the patient/family, an appropriate advice to the
family/system based on the assessment, the analysis of the care situation and the
conversation with the patient and family (including technological possibilities)
Case
The patient is a 61-year-old man with adenocarcinoma colon as the main diagnosis. The
patient has been experiencing abdominal pain since the end of 2022. Has gone to the doctor
(puskasmas) for this. Here Mr. was told that it was gastritis and that it would go away on its
own. Because the pain persisted, the patient's wife took him to the hospital.
In the hospital with a CT scan, it was found that Mr. had a tumor in the abdomen. The tumor
was removed by a laparotomy on January 14. When the tumor was removed, part of the
intestine was also removed and Mr. received an ileostomy.
At the beginning of April, the problems started again. Mr. experienced abdominal pain and
pain around his stoma. On April 13, Mr. admitted with these complaints and a CT was done
again. On this CT scan it was found that Mr. had another tumor. Subsequently, a new
