What does licensure, certification, and accreditation mean in regards to a
healthcare facility?
Licensure is required and usually by the state government.
Certification is voluntary and usually provided by a professional organization.
Accreditation is not necessary and is voluntary. Accreditation is usually provided by an
external organization.
Hospital Committees and their function/purpose.
1. Credentials Committee - essentially the personnel committee for the medical staff.
Advisory in nature.
2. Executive Committee - highest committee level of the medical staff. It has the power
to act for the medical staff and make decisions between formal meetings.
3. Joint Conference Committee/Function - Serves as a liaison between the Governing
Board, the Chief Executive Officer of the hospital and the Medical Staff. Provides
opportunity for discussion.
4. Infection Control Committee - responsible for overseeing the infection control
activities of the institution. Responsible for reducing/eliminating hospital acquired
(nosocomial) infections.
5. Medical Record Committee - reviews the quality of patient records in the hospital.
6. Mortality Committee - reviews deaths and patterns of death.
7. Pharmacy and Therapeutics Committee - responsible for dealing w/ pharmacy
policies and practices.
8. Blood Utilization Committee - monitors blood ordered, used and wasted. Also
monitors blood transfusion reactions.
9. Surgical Case Review Committee - AKA tissue committee. Reviews surgeries and
unnecessary surgeries. Focuses on pre-operative, post-operative and pathological
diagnoses.
10. Quality Assurance Committee - oversees/coordinates the overall quality
improvement efforts of the hospital.
11. Utilization Review Committee - reviews cases for the medical necessity of the
admission, treatment and length of stay.
12. Risk Management Committee - develops policies and procedures to enhance a safe
environment.
13. Forms Committee - approve the design of forms used in the medical record.

Assisted Living Facility - provides housing and personal services to those unable to live

Resident Care Facility - pts. unable to live independently. AKA Independent Living
Facility/Retirement Center.
OTHER CARE SETTINGS
Hospice Care - support to terminally ill pt and families.
Home Health Care - provides health, social and other supporting services to individuals
in their home. Must be ordered by physician.
Ambulatory care - Health care services provided in an outpt setting. Ex. PCP,
freestanding surery centers, walk-in clinics.
Hospital Outpt - Pt comes to the hospital for svcs but is not admitted as inpt.
Hospital Ambulatory - Clinics, ambulatory surgery, lab, radiology, therapy and
pharmacy.
OUTPT CLASSIFICATIONS - 3 Categories
Emergency Otpt. - Treatment of a condition that requires immediate service (Life/Death
situations)
Clinic Outpts. - When a clinic holds regularly scheduled clinics.
Referred Outpts. - pt comes to receive exclusive services (diagnostic or therapeutic

the diagnosis recorded at the end of the H&P, but before any tests/studies.
Differential Diagnosis
At conclusion of H&P, several diagnoses in mind.
