HPI: The patient is a 69-year-old Caucasian male accompanied by his wife to the clinic due to

the patient’s difficulty with balance, and his walking is becoming slower. The wife reports the

patient has a history of a right-hand tremor for several years. Mr. Tony is a right-handed man

who says he has noticed that his Parkinson’s Disease symptoms are getting worse. It was seen

more during rest and got aggravated during movements. Recently, he reported the lower

extremities were also affected. The patient also gives a history of difficulty in walking in the

form of difficulty starting, and once he starts walking, he stoops forward with small and fast

steps. However, he freezes during turning and tends to fall forward and backward. The onset was

last week, and he has been getting worse despite faithfully taking his medications. He takes

Sinemet, and he self-adjusted the medicine, going from four times a day to six times a day, with

only satisfactory improvement. His tremor is unilateral, and he also has muscle rigidity. He said

he has been falling a lot lately and has increased difficulty getting out of a chair. He also reports

his gait is very unsteady, and he uses a rolling walker, but reports do not help. He has a past

medical history of Parkinson’s disease for ten years, HTN, CHF, and GERD. He denies any chest

pain, headache, dizziness, or palpitations. He denies any fever, chills, cough, nausea, vomiting,

or diarrhea. He denies any changes in bowel or bladder regimen. He denies any seizure, syncope,

or head trauma.

3. Omeprazole 20 mg orally daily (GERD)

7. Ubiquinone one tablet orally daily

8. Melatonin 3 mg orally at bedtime (Sleep aid)
