Assessing the Problem: Leadership, Collaboration, Communication, Change
Management, and Policy Consideration
Non-Medication Adherence Among Hypertensive Hispanics
Hispanics have the least control rates (34%) for blood pressure (BP) in the United States compared to Whites (53%) and Black populations (43%) (Gu et al., 2017). The Hispanic community has significant gaps in the treatment and control of hypertension (HTN) among Hispanics of Dominican, Southern, and Central American descent (Gu et al., 2017). Hispanics aged between 45 and 74 years have a higher prevalence rate of HTN than whites (Gu et al., 2017). Hispanics are experiencing a greater rate of increase in HTN-related mortality than other ethnic groups due to a mix of these factors (low rates of control and growing incidence rates).
Hispanics' poor control of blood pressure may be due to medication non-adherence. Compared to black and white people, hypertensive Hispanic individuals reported the lowest adherence to their prescriptions. In addressing the racial/ethnic differences in HTN management, treatments that seek to improve compliance in this high-risk population are needed urgently.
The Importance of the Study To A Baccalaureate Degree
Health providers have a growing interest in better understanding factors related to hypertensive Hispanic patients to prevent medication adherence discrepancies and enhance health outcomes. Nurses are exceptionally prepared to influence medication adherence by addressing concerns on medication regimens such as timely medication intake, even though nurses frequently face intrinsic patient barriers such as patient illiteracy and personal habits and attitudes toward health. 2
Nurse-led treatments that include drug therapy management can increase adherence in
Hispanic populations. Patient-centered medication adherence approaches promotion is intended to educate and give more insight into the purpose and need to adhere to the therapy (Ndefo & Zagaar, 2017). High-risk communities, such as immigrants of Hispanic descent, are
more likely to have better outcomes from culturally competent nurse interactions that encourage adherence to medical treatment through traditional, cultural and social support systems. According to Ndefo & Zagaar (2017), Hispanic patients who have a telephonic motivational conversation are likely to adhere better to their medications.
Furthermore, Hispanics who are less acculturated and with poor health literacy need more specifically targeted interventions to enhance adherence. Improved Spanish language competence and an understanding of the social structures that underpin the cultural ideas and values of Hispanics are critical initial steps for nurses in improving adherence perceptions within this demographic (Ndefo & Zagaar, 2017). A program based on health-promoting and includes nurses can successfully encourage proper medication usage among underserved Hispanics by addressing hurdles related to adherence through education and social support.
Review of the Literature and Data Reliability
This Capstone sought to find literature on evidence-based strategies in helping Hispanic patients lower their blood pressure. Using different databases, including CINAHL, Public Medline (PubMed), ERIC, Google Scholar, and ScienceDirect, a literature review yielded several articles. The Hispanic population, hypertension, blood pressure control, blood
pressure management, health coach, coaching, teach-back, education, cultural competency, minority, low income, access to care, and linguistic barrier were some key terms used in the search. The search did not include the terms heart failure and COPD to verify appropriateness. The selection of articles criteria was peer-reviewed studies published between 2016 and 2022 based on context, level of evidence, and relevance. The search
