tracker software (208), submit dept. application and pay fee (208), provide complete physical (updated

Explain the class attendance policy. ANS: Absences are unexcused until a absence report from Student
Services is completed with supporting documents. If a student is to be absent on an exam date, they
must contact the instructor BEFORE class time. Failure to do so will result in an unexcused absence and a
grade of 0 on the exam.
Explain the testing policy. ANS: No watches allowed, and all information on the exams is the property of
the College of Nursing.
Explain the tardy policy. ANS: After three episodes of tardiness in the classroom, the student will receive
an unexcused absence.
Explain the cell phone policy. ANS: Students must turn off all cell phones during lectures; cell phones
may be used for ONE specific purpose: obtaining information relevant to the student's clinical
,assignment, only in designated areas- NOT in the patient care areas. Personal cell phone use is limited to
during lunch breaks. DURING TESTS cell phones must be given to the exam proctor.

withdrawals ANS: Allowed 1 fail/withdraw from a nursing class, after that the student can no longer
major in nursing at ULL.

investigation? ANS: A Level I background check is required for admission to NURS 208; If a student is
licensed or a certified caregiver, a Level II background check is required.
What are the grading policies? ANS: Test and quiz grades will be recorded to two decimal places.
Students must achieve a 77.00 in order to be successful in the course.
Describe student grading and progression. ANS: In clinical courses, a student must pass both the clinical
and theory compenent in order to pass the practicum.
,What is the make-up exam policy? ANS: With an excused absence, the make up exam must be taken
within 5 weekdays of the day of return to classes.
What is the absence / tardy policy? ANS: A student can be dropped from or fail a course for absences
over 10% of the total course hours. An absence is excused only in the event of : documented illness,
documented evidence of the death of an immediate family member, approved UL activities, or other
situations approved at the discretion of the BSN coordinator.
Explain drug and alcohol screening. ANS: Drug and alcohol screenings are a requirement of many clinical
agencies. Screenings are required under circumstances of : enrollment into clinical courses, randomly
during clinical courses, any suspicions of substance abuse by a student, or a student involved in a
medication handling discrepancy. Students are responsible for any expenses incurred related to testing.
What are the health requirements for clinical courses? ANS: - An annual history and physical
examination documenting absence of health problems. -A negative skin test for tuberculosis. - Proof of
annual flu vaccine. -Documentation of 2 MMR vaccines. -Documentation of TDAP. -Proof of three series
Hep B vaccine. -Varicella antibody testing. -A list of all prescribed medications.
Explain the appeals process. ANS: A letter with supporting extenuating circumstances can be addressed
to the Chairperson of Appeals Subcommittee of the Semester Coordinators' Group- who along with the
committee will make a recommendation to the Dean of the CONAHP, and the Dean will make the final
