It is estimated that by 2025, more than 1.5 billion individuals will suffer from
hypertension worldwide, this represents approximately 50% of heart disease risk and 75% of
stroke risk (Himmelfarb, Commodore-Mensah, & Hill, 2016, p.243). Even with the increased
prevalence of hypertension, 25% are unaware they have hypertension and almost 30% do not
receive care for their diagnosed hypertension (Himmelfarb, Commodore-Mensah, & Hill, 2016,
p. 243). Not only is hypertension a major chronic disease, but it is the most important risk factor
for other co-morbidities such as cardiovascular disease, renal disease, stroke, and premature
death. Managing hypertension can be overwhelming and many patients face difficulties in
disease management. This paper will analyze the impact of healthcare technology, care
coordination, and community resources on patient’s with hypertension.
Impact of Healthcare Technology
E-health is defined as digital or electronic technologies used in medicine and more
specifically the electronic processes and communication technologies utilized to improve the
quality of care and provide efficient healthcare to patients (Omboni, Caserini, & Coronetti, 2016
p.187). The most popular of the e-health services are telemedicine and mobile health (m-health)
(Omboni, Caserini, & Coronetti, 2016 p.187). Telemedicine allows for the exchange of medical
information via electronic communications and the ability to diagnose and care for patients at a
distance. While m-health is the use of mobile devices to communicate between doctors and
patients. Both telemedicine and m-health are widely utilized to broaden and improve the care
