As I reflect over the past seven weeks, I was able to achieve both clinical and classroom
activities that allowed me to grow stronger and more confident in decision making as it is related
to obtaining my practitioner degree. This allowed me to provide high quality, safe, patient
centered care grounded in holistic health principle.
The Patient health questionaries’ (PHQ-9) is a tool used to monitor the severity of depression and
is valid to use in the primary care setting. This tool can be used to make a tentative diagnosis of
depression in the at-risk population. Upon assessment Rakel, & Rakel, (2016) states “the
integration of the tool in clinical practice may enhance care and improve clinical outcome”.
Depression screening do not diagnosis depression, but it can provide critical information
regarding the severity of the patient symptoms at any given time.
Another example: in the clinical setting, a male patient present with a score and PH9 score of 18.
Upon assessment we learned that the patient works a night shift and has custody of his 8-year-old
son. The patient acknowledged about 1 year ago that symptoms started after his ex-girlfriend lost
their baby when she was 5 months pregnant. The patient expressed he hears voices screaming at
him and telling him different ways to kill himself. He was unable to stay at work and due to the
intensity of the voices. The patient was tearful however, when we begin to talk about his son and
family support, he admits this is the reasoning he came into the clinical to receive help. Although
the patient present with a high PH9 score the patient does not have an active plan in place and
has a great family support. The preceptor and I was able to consult in office with the MD and
found that the best option is to give sertraline 25 mg and stat referral to a counselor.

Integrate clinical prevention and population health concepts in the development of
culturally relevant and linguistically appropriate health education, communication
strategies, and interventions.

Prior to week 6 I did not have a clear understanding of post-menopausal and sexuality issues in
the older adult. I then learned that Genitourinary syndrome of menopause (GSM) is an issue that
the pre-menopausal and post-menopausal women face. Upon research Faubion Sood & Kapoor
(2017) states “High concentrations of estrogen receptors in the vaginal mucosa, vulva, bladder,
urethra, and pelvic floor.” The vagina mucosa becomes thin, causing vaginal drying and
dyspnea. Most women feel embarrassed to talk about sexuality concerns, or they may adopt an
attitude that if the provider does not talk about it then they do not want to talk about it. Exploring
options to get women talking about their sexual health and not feeling to embarrassed is a great
way to start with the woman confident in understanding. I found that providers can have a
decrease sexual desire is a screening tool that ask 5 questions about the women’s the women’s
sexual desires and interest.
